Better Care Together is our health and social care transformation programme. It focuses on ensuring that individuals and communities can receive the majority of support they require from within their neighbourhood. This includes a strong focus on prevention and staying well - enabling individuals to achieve the outcomes that matter most to them.

The delivery of an enhanced Primary Care Team is one initiative being tested as part of our approach, and an exciting opportunity has arisen to be part of this new and unique team based in Tilbury and Chadwell. These posts will be based in General Practice, working with existing primary care teams to triage, manage, and follow up patients registered with GPs in Tilbury and Chadwell. This locality is undergoing significant development and is easily accessible by car, train and other public transports. There are 8 practice sites in Tilbury and Chadwell covering a total population of approximately 35,000 patients.

General Practice in Thurrock is evolving, with practices coming together across the localities to manage their population, in line with the Better Care Together vision. This innovative approach allows for new roles to complement the existing traditional General Practice roles to manage more patients at the point of contact. The team’s skill mix has been designed based on a local population needs assessment, and will consist of physiotherapists, clinical pharmacists, advanced nurse practitioners, physicians assistants, community psychiatric nurses, and paramedics.
NELFT is working closely with Thurrock CCG and General Practices regarding this new initiative. Staff will be contracted by NELFT and accountable to the General Practices.
